Taking Control of Per Diem Management: The Ups, Downs, and Benefits of Per Diem Redesign at CAMC

The use of Per Diems, while a central part of most staffing structures, can be riddled with challenges. Such was the case at Charleston Area Medical Center when they took a bold step to re-examine their Per Diem program and move their organization in a new direction. Recognizing that the systems were out of balance, CAMC traced many of the problems back to a well-intentioned incentive program that quietly transformed from a “temporary” solution to a way of operating that was compounding the problem. However, navigating through the challenges, nervousness and uncertainty of what would happen when trying to shift a program that had become deeply embedded into the culture has been and remains a big mountain to climb. A new system implemented as a trial in one 34-bed Telemetry Unit has charted a new path to effective Per Diem use for CAMC with the results that have been achieved including a 36% average increase in RN fill rates and a jump in Per Diem commitments from 19 to 117 hours in 6 months.
